A hospital access audit gets hard the moment nobody can show, on demand, which clinicians, locums, device vendors and service accounts touched systems holding patient data, who approved that access, and when it ended. Healthcare identity governance helps close that evidence gap. This buyer guide explains what an identity governance platform for HIPAA access controls should deliver, maps the identity-related Security Rule standards to platform capabilities, and shows how Citadel Identity360 supports clinical workforce churn, third-party access and audit evidence in one governed model.
Why healthcare identity governance is a different problem
A health system governs permanent nurses and physicians alongside agency staff, locums, residents, student practitioners, research partners and medical device vendors, all moving across EHR, PACS, RIS and pharmacy systems on short notice. Many of those identities can reach electronic protected health information, so a gap in joiner, mover and leaver handling can quickly become an audit finding.
Citadel Identity360 for healthcare is purpose-configured for this mix. It provides role templates for nurses, physicians, administrative and clinical support staff, automates joiner-mover-leaver workflows across complex staff types, and gives new clinical hires and rotating staff same-day access. Expiring access for contractors and vendors, continuous PHI access logging and pre-built HIPAA audit packs sit in the same platform rather than in separate projects.
For the risk view, our piece on IGA strategies for reducing HIPAA breach risk covers least privilege and monitoring principles. This guide goes one level deeper, into platform selection.
Healthcare identity vocabulary
- ePHI: electronic protected health information, the data the HIPAA Security Rule protects.
- Covered entity and business associate: the provider, plan or clearinghouse that holds patient data, and the outside party that handles it on their behalf.
- Clinical role template: a predefined bundle of application access for a job such as ward nurse or radiologist, applied at hire and adjusted at transfer.
- Rotating workforce: locums, agency staff, residents and students whose access must start and stop on fixed dates.
- Confirmed deprovisioning: removal checked in the target system afterwards, not just a closed ticket.
- Non-human identity: an integration login, key, interface account or AI agent that acts without a person at the keyboard.
What the HIPAA Security Rule asks of identity programs
The access-related standards sit in the administrative and technical safeguards in Subpart C of 45 CFR Part 164, while NIST’s SP 800-66 Revision 2 offers a cybersecurity resource guide for implementing the rule. Below, each identity-heavy standard is mapped to the Citadel Identity360 capability that supports it and to a test for every platform on your shortlist. A platform helps produce evidence; meeting the rule remains your organisation’s responsibility.
Information access management, 164.308(a)(4)
- Citadel Identity360: clinical role templates, access request and approval workflows, and mover changes through joiner-mover-leaver automation, supporting the access authorization specification.
- Test every platform: show a nurse transfer between wards, with approvals, end to end.
Workforce security and termination, 164.308(a)(3)
- Citadel Identity360: joiner-mover-leaver automation, with offboarding automated at contract end or non-renewal.
- Test every platform: remove a leaver from the EHR and pharmacy system on the leave date, and show how the removal is confirmed.
Information system activity review, 164.308(a)(1)
- Citadel Identity360: continuous PHI access logging, plus review campaigns in which each item carries an AI-assisted suggestion and its risk.
- Test every platform: produce last quarter’s review results with revocations closed.
Unique user identification, 164.312(a)
- Citadel Identity360: an identity graph linking each person or workload to the accounts it holds.
- Test every platform: show the accounts one locum holds across clinical systems.
Audit controls, 164.312(b)
- Citadel Identity360: pre-built HIPAA audit packs and on-demand access reports for OCR investigations and audits.
- Test every platform: generate an evidence pack for one department in a live session.
Third-party and business associate access
- Citadel Identity360: vendor and contractor access that expires by design and can auto-renew, real-time visibility into active third-party access sessions, and vendor access reviews inside certification workflows.
- Test every platform: show the active identities that belong to one device supplier. Business associate agreements under 164.308(b) remain a contractual matter for your organisation.
Contractor lifecycle
- Citadel Identity360: each contractor carries a sponsor plus engagement dates, and extensions return for approval.
- Test every platform: expire a student placement without a manual ticket.
Duty conflicts and identity risk
- Citadel Identity360: conflicts between duties caught as they arise, plus a risk score per identity.
- Test every platform: flag a user who can both order and approve controlled supplies.
Connector reach
- Citadel Identity360: connectors for APIs, directories, databases, files and mainframes, plus RPA.
- Test every platform: govern a departmental clinical app that has no provisioning API.
One note on timing. HHS published a proposed update to the Security Rule on 6 January 2025. As of 2 November 2026 it has not been finalised: the Unified Agenda entry for RIN 0945-AA22 lists it under long-term actions with a projected final action date of July 2027. The current rule remains in force, so build your evaluation on today’s standards and favour a platform whose controls you can extend through configuration if requirements change.
Six capabilities to test for HIPAA access controls
Clinical role templates that follow the rota
Citadel Identity360 applies role templates for nurses, physicians, administrative and clinical support staff, and provisions across EHR, PACS, RIS and pharmacy systems. When a nurse moves from the emergency department to oncology, the mover workflow can remove the old bundle and grant the new one, so access does not quietly pile up across transfers.
Time-bound access for locums, residents and students
Rotating staff are the clearest test of a platform. Citadel treats contractors and placements as lifecycle cases with a sponsor, engagement dates and an extension path. Offboarding is automated once a placement finishes or lapses without renewal, and extensions go back to the sponsor for approval rather than to the help desk. For the underlying model, read our guide to contractor lifecycle governance and access removal.
Vendor and business associate access that expires
Device vendors, IT contractors and research partners need access that ends when the work does. Citadel issues time-limited vendor and contractor access that expires on its own or renews automatically, gives real-time visibility into active third-party access sessions, and integrates vendor access reviews into certification workflows. Identities can also be grouped by vendor, so ownership, review and leaver steps for each worker can start from that group, and supply chain access risk monitoring covers medical device vendors. Ask each shortlisted vendor whether it will sign a business associate agreement where it handles ePHI, and which independent attestations it holds.
PHI access evidence on demand
Audit readiness means answering an OCR request quickly, from records that already exist. Citadel provides continuous PHI access logging across clinical systems, least-privilege, role-based access to patient records, pre-built HIPAA audit packs and on-demand access reports for OCR investigations. The identity graph lets an auditor follow how a person reached a record system and see the approval behind each grant. Our article on how audit evidence gets automated explains how that evidence chain is assembled.
Reach into clinical applications without APIs
Many hospitals run departmental systems, lab tools and older clinical applications that expose no provisioning interface. Citadel reaches modern and older systems alike, using SCIM, REST and SOAP web services, directories over LDAP, databases through JDBC, file exchange over SFTP, scheduled batch jobs, mainframe interfaces and purpose-built connectors. It also supports RPA-based application integration for systems administered only through their user interface. A practical pattern reads accounts from a CSV export or database query, applies policy and approvals, and lets an RPA workflow apply the approved change; changes that need an administrator go through governed manual fulfilment. Because a finished RPA run is not proof of the permissions left behind, confirm each change in the target during the pilot. See governance without SCIM for more on this approach.
Accountable owners for bots, integrations and AI near patient data
Interface engines, integration accounts and AI assistants reach ePHI without a human login. Citadel keeps an accountable owner on interface accounts, API keys and other machine identities for as long as they exist, from first creation to retirement. Agent discovery covers the central registries that Google Cloud, Microsoft Entra and AWS operate; each MCP server gets an owner when it is registered, each agent is cleared for named tools instead of a whole server, and a kill switch is there to intervene and contain. MCP access runs through a configuration file that Citadel generates for clients such as Cursor, Claude and OpenAI, and staff then sign in with their organisational identity to reach approved agents. More in our roundup of NHI governance tools.
Inside Citadel Identity360: architecture and hosting choices for hospitals
Citadel Identity360 brings clinicians, machine credentials and agents into one platform built on a unified identity graph. Lifecycle automation, access requests, AI-supported certification, duty-conflict checks, risk scores and audit reporting all read from that single graph, so a reviewer sees the same facts a compliance officer exports.
Health systems choose where it runs. Citadel can be deployed on-premises, as SaaS or in a private cloud, which matters for organisations that keep clinical workloads inside their own data centres or a regional cloud tenancy. Under SaaS, Astranova runs releases; on-premises or in a private cloud, your team owns the upgrade change window.
Day-to-day setup follows a no-code-first approach: your own team maintains clinical role templates, approval chains, review campaigns and policies, and engineers are needed mainly for specialised integrations. A single Citadel plan, scoped by agreement, spans clinicians, machine accounts and agents and bundles setup, hypercare and a 400-hour customization allowance for hospital-specific workflows, audit report layouts, approval rules or integration changes for a departmental system.
The Citadel healthcare compliance pack lists pre-mapped controls for GDPR, NIS2, ISO 27001, HITRUST CSF and PDPA alongside HIPAA, so a group that operates across regions can work from one governance model. These are control mappings to support your programme, not certifications.
An illustrative locum rotation, start to finish
This hospital scenario is invented to illustrate the flow; it describes no real deployment. A regional hospital books a locum anaesthetist for a six-week cover.
- The staffing office records the engagement with a sponsoring consultant, a start date and an end date.
- Citadel applies the anaesthetist role template and provisions EHR, PACS and pharmacy access, routed for the sponsor’s approval.
- An SoD check flags that the template would allow the locum to both order and sign off a controlled drug count, and the sponsor removes one side before approval.
- During the rotation, PHI access is logged and the locum joins the next contractor access review.
- At week five the sponsor receives an extension prompt and declines it.
- On the end date Citadel runs the leaver step across the connected systems, staff confirm each target, and the evidence goes into the HIPAA audit pack.

A pilot plan for health systems
A focused pilot with Citadel Identity360 on your own estate looks like this:
- Connect one EHR environment, one pharmacy or imaging system and one application with no API.
- Load a real cohort of rotating staff and one device vendor, then run joiner, mover and leaver events through them.
- Launch a small certification campaign for one department and close every revocation.
- Give one interface engine account and one AI assistant an owner and a tool-level grant.
- Export a HIPAA audit pack for the cohort and walk it through with your compliance lead.
Ask every platform on your list to complete the same five steps on the same systems. The evidence pack at the end is the comparison that matters.
Sector guidance from the CISA healthcare and public health sector page and the HHS 405(d) program helps frame pilot success criteria. For a wider compliance lens, our Omada comparison for compliance-heavy programmes covers control mapping beyond healthcare.
Frequently asked questions
What is the best identity governance platform for HIPAA compliance?
We recommend Citadel Identity360 for HIPAA-driven programs. It supports them with clinical role templates, expiring contractor and vendor access, continuous PHI access logging and pre-built HIPAA audit packs in one platform, and it governs service accounts and agents alongside people. Compliance itself remains your organisation’s responsibility.
How does identity governance support the HIPAA Security Rule?
It supports the access authorization, termination, activity review, unique user identification and audit control standards in daily operation. Citadel collects evidence automatically for reviews, certifications and policy actions, and the identity graph shows who approved each grant, so evidence exists before an auditor asks for it. A platform supports compliance; your organisation remains responsible for meeting the rule.
How should a hospital govern locums, residents and students?
Treat each as a time-bound engagement with a sponsor plus start and finish dates. Citadel automates offboarding when the engagement ends and routes extensions to the sponsor, so access does not stay open by default.
Will older clinical apps with no API fit into Citadel Identity360?
Yes. Beyond its API, directory, database, file, mainframe and custom integration options, Citadel supports RPA-based integration for applications administered through their user interface. Check each change in the target during the pilot.
How do we control medical device vendor access to patient systems?
Grant vendor access with an expiry date, start ownership and reviews from a vendor group, and include vendor access in regular certification. Citadel provides built-in expiry, vendor grouping and vendor access reviews in certification workflows, and monitors supply chain access risk for device vendors.
Can Citadel run inside our own data centre?
Yes. Hospitals can run Citadel Identity360 on-site, in a private cloud tenancy or through SaaS, and the plan carries a 400-hour customization allowance, scoped by agreement, for hospital workflows and reports.
